Our services include the regular assisted living services such as assistance with ADLs. Path Of Life also offers the following unique services to its community:

Path Cares Complimentary Evaluation –

An evaluation is a tool used to obtain information of a resident in order to create an individualized plan specifying areas of need and attention, goals, and tasks that will be communicated to staff and the resident’s team. Your evaluation may be done in the privacy of your own home. Each evaluation is individualized and performed by an experienced registered nurse.

Path Of Life Enrichment Program –

Our program is specific to each individual’s interest and talents. Reinforcement and encouragement as well as plenty of smiles and positive, life filled communication enhances the lives of all our residents. Enriching each individual promotes well being, security, trust, and further participation.

Home Visit Services –

Our complimentary home visit is done by a registered nurse. The visit allows a complete evaluation to make visible the needs of a resident and caregiver in order to connect to the appropriate resources and create partnerships with services that exist within the resident’s community.

Personal (One on One) Transitioning –

Our staff understands how sensitive the transition period for each resident can be, therefore we personally meet with family and resident and staff to get involved and build a bond from the very beginning to achieve the best and most comfortable transition period that we can provide.
